The following objects, receipt of which on , 194....., is acknowledged by Prof. Alfred Bellinger Yale University , the undersigned, are lent by the University Museum to the undersigned for a period of 3 months from April 1st, subject to extension of time, in consideration for which loan the undersigned agrees, in the event of the failure of the undersigned (whether or not resulting from the negligence of the undersigned) to return any such object to the University Museum within 5 days after the expiration of said loan period or any extension thereof, to pay to the University Museum the amount set forth opposite the following description of such object; in addition THE UNDERSIGNED AGREES TO BE BOUND BY THE CONDITIONS PRINTED ON THE REVERSE OF THIS AGREEMENT:
